PLEASE NOTE:  If you wish to make a claim or to exclude yourself from the class (that is, to “opt out”), you must file the appropriate form (the claim form or the opt-out form) with the Claims Administrator on or before the June 7, 2004 deadline.

Papers must be postmarked by June 7, 2004, if sent by U.S. mail, and must actually be received by June 7, 2004, if sent by any other means.

If you file neither the claim form nor the opt-out form, you will lose your rights and will receive nothing from the settlement.  Call or write to Plaintiff’s Class Counsel if you have questions.
